2/20/2023 EnchantaVaughn Approved Behavior: Anthony was active and engaged when QMHP-C arrived at the residence. Anthony was very polite and communicated happy thoughts around his family. Anthony participated in his therapy without getting frustrated or acting out. Anthony was receptive to QMHP-C prompts and directives. Anthony was open to sharing his feelings and the things he believed he needed assistance with. Behavior: Anthony displays understanding of his strengths and weaknesses. Anthony mood was viewed as patient and laid back. Anthony was in tune with personal boundaries activity, due to giving others space and knowing when to be assertive. Anthony admitted that he disliked when people invaded his privacy and space, so it was easy to respect other’s space. Anthony was happy to know that QMHP-C was mindful of how close/far they sat from each other and making sure he felt safe at all times. Anthony did not view any behaviors that could be perceived as negative. Intervention: QMHP-C conducted the session to introduce activities that focused on parenting and journaling. QMHP-C reviewed with Anthony what trust means and why it is important to be honest and trustworthy. QMHP-C actively listened and provided support to Anthony when needed. QMHP-C inquired about how Anthony was feeling. QMHP-C made sure Anthony was comfortable and in a good space before introducing him to a new therapeutic skill. QMHP-C revisited effective communication skills and positive verbal affirmations with Anthony to help him express himself in an open way. Intervention: QMHP-C went through program overview with Anthony and mom to reiterate key points and program objectives that would pertain to Anthony specific wants and needs. QMHP-C retrained Anthony on relaxation skills, which included deep breathing, muscle relaxation, and calming imageries as a way for Anthony to regain and control his rebellious and hyperactive conduct. QMHP-C reiterated to Anthony that he doesn’t have to be so hard on himself and that he should give himself some credit for surviving a life crisis. Response: Anthony struggled with expressing his feelings in response to the negative energy around him. Anthony frowned his face up and ignored QMHP-C prompts when directed to follow his mother’s directions. Anthony failed to identify positive ways to deal with his anger and frustration, remaining aggravated the entire session. Anthony continued to express negative statements to QMHP-C. Anthony reduced eye contact with QMHP-C and showed no interest in following safety plan. Anthony offered no form of verbal communication and refused to calm down. Response: Anthony participation level was at an all-time low. Anthony looked confused, indicating that he didn’t know the reason for his negative behaviors. Anthony shared some of his needs. Anthony was open to new interventions and promised to let his mom and counselor know when he felt overwhelmed and needed a break/cool down from everything.
